Is Toxoplasma gondii infection a risk factor for leukemia? An evidence-based meta-analysis

Background: Possible associations of parasite infection with cancer risk have recently attracted much attention. Published studies concerning the association between Toxoplasma gondii (T. gondii) infection and leukemia risk have generated inconsistent results. In the present study, we aimed to address this topic by conducting a quantitative meta-analysis. Material/Methods: Relevant publications were searched in electronic databases and eligible studies were rigorously screened and selected. Essential information was extracted and the data were pooled. Subgroup analysis on source of controls and detection target was also performed. Results: A total of 6 studies that met the inclusion criteria were selected. The overall data show that T. gondii infection might have an association with increased leukemia risk (OR=3.05; 95% CI=1.83-5.08). Similar results were shown in the subgroups regarding source of controls and detection target. Conclusions: Our results suggest that T. gondii infection might be a risk factor for leukemia, providing new insight into the etiology of leukemia. Future studies with large sample sizes in different geographic areas are needed to confirm this conclusion.

Background: Toxoplasmosis is one of the most important diseases in humans and animals. Almost one-third of the human population around the world is infected with toxoplasmosis. The agent of this parasitic disease is a protozoan called Toxoplasma gondii (T. gondii) that causes encephalitis in people with suppressed immune systems and abortion, mental retardation and chorioretinitis in the fetus. Alzheimer's disease (AD) is the most important neurodegenerative disease. Objectives: Due to the high prevalence of toxoplasmosis in Iran and evidence on its impact on neurodegenerative diseases, this study was performed to evaluate the T. gondii infection in patients with AD. Patients and Methods: In this case-control study, after selection of alzheimer's patients (APs) referred to Imam Reza psychiatric hospital of Khorramabad, west of Iran, and healthy controls (each group consisted of 87 individuals), using the convenience sampling method and under the supervision of a neurologist, blood samples were taken during July 2014 and January 2015. The collected samples were transferred to the research laboratory of parasitology under cold chain storage and then, the serum samples were separated by centrifugation and were frozen at -20 degrees C until use. The T. gondii IgM and IgG specific antibodies were assessed in serum samples using commercial Enzyme Linked Immunosorbent Assay (ELISA) kits. Results: The overall prevalence of T. gondii infection in patients with AD and the control group, using ELISA assay, was obtained as 66.6% (58/87) and 56.32% (49/87), respectively (P = 0.99). In this study, there was no significant association between T. gondii infection and AD. On the other hand, no statistically significant difference was observed between the two groups in terms of infection with T. gondii (P = 0.99). Conclusions: Higher prevalence of T. gondii in patients with AD compared to controls showed the possible impact of this parasite in AD, which may exacerbate symptoms, and this requires special attention of specialists and patient families.

Context: Toxoplasma gondii is a protozoan parasite with a world-wide distribution. However, the majority of infected cases remain symptomless. There are raising scientific evidences indicating that parasitic infections induce antitumor activity against certain types of cancers. The inhibitory effect of T. gondii on cancer growth has also been shown in cell culture and mouse model. Aims: Considering the anti-tumor effect of this parasite, in this study the relationship between low titer of antibodies against T. gondii and resistant to cancers has been investigated. Subjects and Methods: In two separate experiments, anti T. gondii antibody was estimated in 150 patients with cancer and 120 normal people. Statistical Analysis Used: Chi-square test was used for data analysis. Results: The frequency of low titer antibody against Toxoplasma gondii in cancer patients was significantly higher than the frequency of low-titer antibody against this parasite in normal people. Conclusions: Exposure to T. gondii may be related to resistance to cancer.

Introduction: Toxoplasma gondii is an intracellular protozoan responsible for up to one-third of the world's population infestation. Diabetes is one of the most silent and threatening disease of the modern time it is constantly increasing in both industrialized and developing countries. This is a case of clinically importance for two reason, firstly it will help clinicians save a broad differential diagnosis when attending to evaluate analogous cases and secondly, it may confirm the role of autologous peripheral blood stem cells (PB-SCs) in enhancing auto-immune response against parasitic infection and in regulating insulin uptake in diabetes mellitus type 2 (DM2). Case presentation: We present a unique case of 83-year-old woman from Argentina presenting with a widespread erythema and urticaria for 5 months and DM2 as underlying condition. She was initially diagnosed with unspecific skin auto-immune disorder. By the time of visit she was complaining of constant diarrhea-constipation and general mental and physical fatigue. Conclusion: This case illustrates that toxoplasmosis can present with just simple disseminated and generalized skin erythema with severe itching and, thus can be confused with similar infectious disease such as Epstein-Barr virus (EBV), cytomegalovirus, cat scratch disease or leishmaniasis. The report emphasizes the need of correct diagnostic procedure in confusing cases, and may help to increase the awareness about the identification of this disease. This case may open to the possibility of a different approach and methodology in treatment T gondii and DM2 through the use of PB-SCs.

Possible role of Toxoplasma gondii in brain cancer through modulation of host microRNAs

Background: The obligate intracellular protozoan parasite Toxoplasma gondii infects humans and other warm-blooded animals and establishes a chronic infection in the central nervous system after invasion. Studies showing a positive correlation between anti-Toxoplasma antibodies and incidences of brain cancer have led to the notion that Toxoplasma infections increase the risk of brain cancer. However, molecular events involved in Toxoplasma induced brain cancers are not well understood. Presentation of the hypothesis: Toxoplasma gains control of host cell functions including proliferation and apoptosis by channelizing parasite proteins into the cell cytoplasm and some of the proteins are targeted to the host nucleus. Recent studies have shown that Toxoplasma is capable of manipulating host micro RNAs (miRNAs), which play a central role in post-transcriptional regulation of gene expression. Therefore, we hypothesize that Toxoplasma promotes brain carcinogenesis by altering the host miRNAome using parasitic proteins and/or miRNAs. Testing the hypothesis: The miRNA expression profiles of brain cancer specimens obtained from patients infected with Toxoplasma could be analyzed and compared with that of normal tissues as well as brain cancer tissues from Toxoplasma uninfected individuals to identify dysregulated miRNAs in Toxoplasma-driven brain cancer cells. Identified miRNAs will be further confirmed by studying cancer related miRNA profiles of the different types of brain cells before and after Toxoplasma infection using cell lines and experimental animals. Expected outcome: The miRNAs specifically associated with brain cancers that are caused by Toxoplasma infection will be identified. Implications of the hypothesis: Toxoplasma infection may promote initiation and progression of cancer by modifying the miRNAome in brain cells. If this hypothesis is true, the outcome of this research would lead to the development of novel biomarkers and therapeutic tools against Toxoplasma driven brain cancers.
