The role of latent toxoplasmosis in the aetiopathogenesis of schizophrenia–the risk factor or an indication of a contact with cat?

We assessed IgG antibody to Toxoplasma gondii in 300 inpatients with schizophrenia (SG), 150 outpatients with anxiety and depressive disorders (PCG), and 150 healthy blood donors (HCG). Seropositivity rates were 60.7% for SG, 36.7% for PCG, and 45.3% for HCG (p<0.001). The seropositivity rate for anti-Toxoplasma IgG antibodies in SG was significantly higher that in PCG (chi2 = 23.11, OR = 2.66, p = 0.001) and HCG (chi2 = 9.52, OR = 1.86, p = 0.002). Among SG, 85% of those who reported close cat contact had IgG antibodies to T. gondii. Close cat contacts were reported by 59% of SG, 6% of PCG, and 9% of HCG (p<0.001). There was a nonsignificant positive association between toxoplasmosis and schizophrenia for people with a contact with a cat (OR = 2.221, p = 0.127, CI95 = 0.796-6.192), and significant negative association between toxoplasmosis and schizophrenia for people without contact with a cat (OR = 0.532, p = 0.009, CI95 = 0.332-0.854). Close cat contact (OR = 2.679, p<0.001), 51-65-year age group (OR = 1.703, p<0.001) and education [illiterate+primary (OR = 6.146, p<0.001) and high school (OR = 1.974, p = 0.023)] were detected as independent risk factors in multivariate logistic regression. The effect of toxoplasmosis on risk of schizophrenia disappeared in the complex model analyzed with multivariate logistic regression. Background: The highly prevalent parasitic infection of humans, toxoplasmosis, can have serious consequences even during an asymptomatic course of the disease. The causative agente of infection is Toxoplasma gondii, which proliferates in the intestine of cats. Risk of infection by direct contact with cats is low, but the infection can occur due to contaminated soil, water, unwashed vegetables and fruits, as well as insufficiently heat-treated meat. Objectives: The aim of this study was to synthesize the current knowledge about infection caused by Toxoplasma gondii in a brief review focusing on latent toxoplasmosis in humans. Materials and Methods: More than hundred articles, studies and reports from scientific journals were considered to summarize the infection's characteristics such as prevalence and symptoms in humans. Results and Discussion: The symptoms of acute toxoplasmosis are either featureless, or non-existent, although, for people with immunosuppression it can be fatal, and for expectants it can result in foetal harm. The detection of the infection is conducted with serology methods, however, only in a few European countries are general screenings present. The asymptomatic carrier state is dangerous as well, because the cysts subsist in the brain of the host, which affects behaviour, the regulation of emotions, and also the level of several hormones. The latent toxoplasmosis can be connected to aggression, anxiety, schizophrenia, bipolar personality disorder, suicidal inclination, depression, and the reduction in intelligence quotient. For the reason of the deterioration of reaction time, the disease increases the risk of traffic accidents. The latent toxoplasmosis also influences personality traits; for women and men the alteration of the personality change differs. There is no data on the current epidemiological situation in Hungary, and not even in Europe. A research regarding the extent of asymptomatic carrier state and about the psychological changes in the Hungarian population has not been conducted so far.

Toxoplasma gondii has worldwide distribution in nearly one-third of the human population. It is a neurotropic protozoan parasite so a potential role of T. gondii infection for some neuropsychiatric disorders was postulated. Patients with psychiatric disorders had high toxoplasmosis se- roprevalence. Limited information about toxoplasmosis seroprevalence in psychiatric patients was known in southern area of Saudi Arabia. The current cross sectional case control study aims at determination of the prevalence of T. gondii IgG & IgM in neuropsychiatric patients in Jazan Province. A total of 162 neuropsychiatric patients from Al-Amal hospital for psychiatric health and 162 subjects without neuropsychiatric manifestations from Jazan General Hospital, Jazan City, KSA. were enrolled in the study. Psychiatric diagnosis was based on the International Classification of Diseases-10 (ICD-10 classification). Serological analysis for latent toxoplasmosis (IgG) and active toxoplasmosis (IgM) was done using Enzyme Linked Immunosorbent Assay (ELISA). Investigations for the association with socio-demographic, clinical and behavioral characteristics in psychiatric patients were also done. The serofrequency of IgG antibodies among neuropsychiatric patients was significantly higher than that of the controls (35.8% vs 14.8%) P = 0.0022. OR 3.2 with 95% CI= (1.4952 to 6.8774). However; serofrequency of toxoplasma IgM antibody between neuro-psychiatric patients and controls was not statistically significant (P > 0.05).,Bivariate and multivariate analysis for socio-demographics and possible associated risk factors showed that contact to cats and/or dogs, eating under cooked meat, and contact to soil were significantly higher in neuropsychiatric patients than controls.

Waterborne outbreaks of Toxoplasma gondii have focused attention on the importance of oocysts shed in the feces of infected cats. Cat feces deposited annually into the environment in the United States total approximately 1.2 million metric tons. The annual oocyst burden measured in community surveys is 3 to 434 oocysts per square foot and is greater in areas where cats selectively defecate. Because a single oocyst can possibly cause infection, this oocyst burden represents a major potential public health problem. The proper disposal of cat litter, keeping cats indoors, reducing the feral cat population, and protecting the play areas of children might potentially reduce the oocyst burden.

Toxoplasma gondii is a parasite of birds and mammals. Cats are the only definitive host and thus the only source of infective oocysts, but other mammals and birds can develop tissue cysts. Although feline infections are typically asymptomatic, infection during human pregnancy can cause severe disease in the fetus. Cat owners can reduce their pets' exposure risk by keeping all cats indoors and not feeding them raw meat. Humans usually become infected through ingestion of oocyst-contaminated soil and water, tissue cysts in undercooked meat, or congenitally. Because of their fastidious nature, the passing of non-infective oocysts, and the short duration of oocyst shedding, direct contact with cats is not thought to be a primary risk for human infection.

Possible role of toxoplasmosis in patients with first-episode schizophrenia

Aim: To determine the possible relationship between toxoplasmosis and patients with first-episode schizophrenia (FES) Materials and methods: Seventy-three subjects with FES (15-54 years old; mean. 23.4 years) and 40 healthy individuals (20-54 years old, mean: 30.3 years) were enrolled in the study Most of the FES patients (90.4%) and the control individuals (95 0%) were male Specific IgG and IgM antibodies to Toxoplasma gondii were investigated by enzyme-linked immunosorbent assay (ELISA), and the total amount of antibodies was investigated with the Sabin-Feldman dye test (SFDT) Results: Compared to the control individuals, study subjects had significantly more toxoplasmosis-like symptoms and more cats in the household; they were less likely to live in apartments and more likely to live in ground-floor houses They did consume more uncooked meat, unpasteurized goat's milk, and chicken eggs; had more contact with soil; and were living less often in urban areas and more often in rural areas. Serum samples from 32 (43 8%) and 25 (34.2%) of 73 patients with FES were seropositive for T gondii when tested by ELISA IgG and SFDT, respectively Out of 40 serum samples from control subjects, 13 (32 5%) and 15 (37.5%) were found positive for T gondii by ELISA IgG and SFDT, respectively In patients with FES, 17 (68%) out of 25 who were SFDT-positive were also positive by ELISA IgG test, while 15 out of 48 (31 3%) SFDT-negative serums were positive by ELISA IgG In the control group, 11 out of 15 SFDT-positive serum samples (73 3%) were also positive by ELISA IgG test, while 2 out of 25 SFDT-negative serum samples were positive by ELISA IgG Conclusion: The present study shows that toxoplasmosis might be associated with first-episode schizophrenia. More studies are needed to prove the association between T gondii infection and patients suffering from schizophrenia

In recent years, the effect of Toxoplasma gondii infection on psychiatric-related aspects has been increasingly recognized. T gondii has a high affinity for brain tissue where tachyzoites may form tissue cysts and persist life long. In recent years, 15 serological surveys about T gondii infection and psychiatric diseases have been carried out in different areas in China. Studies showed that the prevalence of antibodies against T gondii in psychotic patients was much higher than in normal persons; statistically differences were significant. Studies also reported that raising cats or enjoying the habit of eating raw or under cooked meet were potential risk factors for the infection of T gondii. The epidemiological and serological evidence support the hypothesis that some psychiatric diseases such as schizophrenia or mental retardation might be linked to T gondii infection.

The clinical sequelae of acute and congenital toxoplasmosis are well established, but that of chronic toxoplasma infection remains uncertain. In rodents, chronic toxoplasma infection is associated with altered behaviour leading to an enhanced risk of feline predation and a putative selective advantage to the parasite. It is proposed that neurotropic cysts of toxoplasma exert an effect on animal behaviour, either directly or via the release of metabolic products. Long-standing toxoplasma infection in humans has been linked to cerebral tumour formation and personality shift. In view of the vast population with chronic toxoplasma infection, further studies of the clinical sequelae of this condition are required.
