The association between Toxoplasma and the psychosis continuum in a general population setting

Background: An increased prevalence of antibodies to Toxoplasma gondii has been found to be associated with psychosis. An association with positive symptom severity in a psychosis risk sample has also been reported. However, the possible association between toxoplasma and positive symptoms in the general population is unknown. Methods: We investigated whether Toxoplasma gondii is associated with psychosis diagnoses and psychotic symptoms in the general population. We used data from Health 2000, a large health survey of the Finnish population aged 30 and older (n = 5906). Levels of IgG antibodies to Toxoplasma gondii were determined and seropositivity defined as a cutoff of 50 IU/ml. Lifetime psychotic symptoms were identified with section G of the Composite International Diagnostic Interview, Munich version (M-CIDI). The CIDI screen was considered positive if the interviewee reported any clinically relevant symptom or at least 3 symptoms regardless of clinical relevance. Lifetime psychotic disorders were screened from the sample and were diagnosed with DSM-IV using case notes and SCID-I interview. The possible diagnoses were: any psychotic disorder, functional psychosis, schizophrenia, other nonaffective psychosis, and affective psychosis. Results: The seroprevalence of Toxoplasma in the population was 13%; 12% of the participants reported at least one psychotic symptom, the most common symptoms being delusions of reference and suspicion, and 4% were considered positive for the CIDI screen. In a logistic regression model, toxoplasma seropositivity was the only significant predictor of the CIDI screen status, when other variables associated with toxoplasma were adjusted for (age, gender, education, region of residence, cat ownership, and C-reactive protein measuring inflammation). Toxoplasma infection was not associated with any psychosis diagnoses. Conclusion: In a large sample representing the whole Finnish population aged 30 and older, we found that toxoplasma infection predicted psychotic symptoms but not psychosis diagnoses. The association between psychotic symptoms and toxoplasma was not explained by inflammation.

Meta-analysis of infectious agents and depression

Depression is a debilitating psychiatric disorder and a growing global public health issue. However, the relationships between microbial infections and depression remains uncertain. A computerized literature search of Medline, ISI Web of Knowledge, PsycINFO, and the Cochrane Library was conducted up to May 2013, and 6362 studies were initially identified for screening. Case-control studies detected biomarker of microorganism were included. Based on inclusion and exclusion criteria, 28 studies were finally included to compare the detection of 16 infectious agents in unipolar depressed patients and healthy controls with a positive incident being defined as a positive biochemical marker of microbial infection. A customized form was used for data extraction. Pooled analysis revealed that the majority of the 16 infectious agents were not significantly associated with depression. However, there were statistically significant associations between depression and infection with Borna disease virus, herpes simplex virus-1, varicella zoster virus, Epstein-Barr virus, and Chlamydophila trachomatis.

Research on infectious agents as a possible cause of schizophrenia has become prominent in the past decade. Toxoplasma gondii has emerged as a prime candidate for a variety of reasons; (i) many studies have reported that individuals with schizophrenia, compared to controls, have a higher prevalence of antibodies to T. gondii, (ii) some individuals with adult toxoplasmosis develop psychotic symptoms similar to those of schizophrenia, (iii) epidemiologically, there are many similarities between toxoplasmosis and schizophrenia, (iv) antipsychotic drugs known to be effective in schizophrenia also inhibit some parasites, including T. gondii, (v) Toxoplasma has been shown to induce elevated levels of dopamine in experimentally infected animals (elevated dopamine is commonly seen in individuals with schizophrenia) and (vi) studies have shown that individuals with schizophrenia, compared to controls, have had greater exposure to cats in childhood. A number of questions remain concerning a role for Toxoplasma in the aetiology of schizophrenia, including the roles of strain variation, the timing and source of infection, and the role of host genes in determining disease susceptibility. The establishment of a firm association between Toxoplasma and the aetiology of schizophrenia and related disorders would represent a major breakthrough in the understanding of these disorders and would lead to novel methods for their treatment and prevention.

Recent meta-analyses have provided a comprehensive overview of studies investigating Toxoplasma gondii antibodies in schizophrenic patients, thus attempting to clarify the potential role these infections might play in causing schizophrenia. Issues for further research have been suggested. Associations and theories that may enrich the current level of knowledge with regard to this significant subject deserve attention. Anti-parasitic agents as well as antipsychotics are effective in treating parasitosis. Both classes of drugs have been shown to exert dopaminergic activity. Parasites and human organisms have a long history of mutual contact. The effect of parasitosis on the host and the host's response to infection are undoubtedly the product of a long evolutionary process. The neurochemical background of delusions of parasitosis is potentially similar to ancient evolutionary traces of altered neurotransmission and neuropeptide gene expression caused by parasites; these include fungal and viral infections. This is very unique in medicine if a class of drugs is effective in the treatment of an illness but also cures the delusion of the same disorder as well. Furthermore, metabolic disturbances such as hyperglycemia and insulin resistance were reported several decades before the antipsychotic era. Toxoplasmosis may also be linked to insulin resistance. Schizophrenia research can benefit from understanding this evolutionary link. New chemical entities that are liable to alter neurochemical changes related to the brain's perception of the risk of predation secondary to parasites may result in new approaches for the treatment of psychosis. These findings suggest that further research is needed to clarify this evolutionary link between parasite infection and delusions of parasitosis. We believe this model may well open up new avenues of research in the discovery of drugs to counteract schizophrenia.
